• Membership dues are non-refundable.
• Family members cannot be substituted for another family member during office visits.
• Your effective date is the day payment is received in full.
• Financial arrangement available through Cherry Financing, subject to eligibility.
• Any cancellations made within 48 hours prior to appointment, and any missed appointments maybe subject to penalty, including possible cancellation of membership plan.
• Cleaning must be performed before the end coverage date.
• This is an exclusive membership plan and NOT dental insurance.
• Co-payment is due at the time of service for any work received.
• Rates are subject to change annually.
• Cannot be combined with other promotional offerings.
• Discount not applied to bleaching services or orthodontic aligners.

• Membership is good only at Family Dentistry at Cherry Hill, therefore, if you are referred to a specialist, discounts offered to members will not apply.

Services Covered for Basic Cleaning plans:

• Unlimited Exams – Includes comprehensive, periodic, and any emergency exams.
• X-rays
• Adult or Child Prophylaxis (Standard Cleaning) – 2 per year* If you require an additional 3rd cleaning it would be an additonal $75).
• Fluoride – 2 per year for children under 16 years old included, additional would be 15% off office fees.
• Sealants - An additional $10 per tooth. 
• Patients on a 3- or 4-month hygiene protocol will receive the value of 2 standard cleanings credited towards their hygiene visits.
• All other services discounted 15% our usual and customary fees.  Services include, but are not limited to:
• Fillings
• Crowns
• Bridges
• Implants and Implant Restorations
• Cosmetic Dentistry
• Root Canals
• Extractions
• Partial/Full Dentures

Annual Membership Dues 2027:

Family Plan Basic (All Fees are based on annual basis):

• First Adult - $350 (Non-Perio Patient, Basic Cleanings) $75 extra for a third cleaning if needed. 
• Second Adult (over 18 yrs. Old) - $300 additional, couple would be $650.
• Family of 4 (2 Adults & 2 Children)-  $999
• Any Additional Adult (Over 18 Yrs. Old) - $300 (If additional adult is Perio patient add an extra $175).
• Any Additional Child (Under 18 years old)- $175

Individual Plans: 

• Adult - $350
• Children - $175

Periodontal Maintenance (Targeted Gum Treatement):

• First Adult -  $475 
• First Year - Routine cleanings, x-rays, exams 
• NO 4 quads of SRP Included, separate charge will incur.
• 15% off all dental work mentioned above.
• SRP (Scaling and Root Planning): $296 per quad , SRP with partial quad : $223 (These are discounted fees for our members, in addition to the annual Perio Maintenence Membership).
